- CVs, Domicile, 5 years Prof DMC / Transcript, House Job Certificate; Experience Certificate (if any), FCPS-I result, 2 years Training complete in Medicine for FCPS-II, CNIC copy. 3 x Passport size photographs, Police Character verification. Those having foreign qualification to submit HEC/PMC equivalency certificate.
